Some providers are taking on patients who do not currently have a primary care provider. Please do not complete this form to be added to the waitlist if you currently have a health care provider (doctor or nurse practitioner) in the region.

Once you have submitted your information, please be patient and wait for a member of our team to get in touch with you. We expect that it may take approximately 6-12 months for us to get to your name on the list. This is an estimate only and our response time may vary.

New patients are accepted on a first-come, first-served basis. We are contacting people on the waitlist in the order that requests were received.

You have recently been diagnosed with cancer and do not have a family doctor or nurse practitioner following your care.

You have a premature baby or newborn/infant who does not have a family doctor or nurse practitioner.

You have received a new schizophrenia or bipolar disorder diagnosis, are in need of medications or require adjustments to your medications for these conditions, and do not have a family doctor or nurse practitioner following your care.
